(I) I/We certify that the information furnished in Declaration of Intent under the Gujarat Micro, Small and Medium Enterprise (Facilitation of Establishment and Operation) Act 2019 and the rules made there under is true, correct and complete to the best of my/our knowledge & belief and no material information has been concealed.

(I) હું/અમે પ્રમાણિત કરું છું/ કરીએ છીએ, કે ગુજરાત માઇક્રો, સ્મોલ એન્ડ મીડિયમ એન્ટરપ્રાઇઝ (સ્થાપના અને સંચાલન) કાયદો ૨૦૧૯ હેઠળની ઘોષણા અને તેના હેઠળ બનાવેલા નિયમો હેઠળ આપવામાં આવેલી માહિતી વાસ્તવિક, ભૂલચૂક વિનાની અને સાચી છે. મારી જાણ અને માન્યતા મુજબ કોઈ જાણકારી ખાનગી/ગુપ્ત રાખવામા આવી નથી.

(II) I/We have read the Gujarat Micro, Small and Medium Enterprise (Facilitation of Establishment and Operation) Act 2019 and the rules made there under and I undertake to abide by them. In case of changes in above details, the same will be intimated to the Nodal agency at the earliest.

(II) હું/અમે ગુજરાત માઇક્રો, સ્મોલ એન્ડ મીડિયમ એન્ટરપ્રાઇઝ (સ્થાપના અને સંચાલન) કાયદો ૨૦૧૯ અને તેના હેઠળ બનાવેલા નિયમો વાંચ્યા છે અને હું તે નિયમોનું પાલન કરવાનું વચન આપુ છે. ઉપરોક્ત આપેલ વિગતોમાં ફેરફારના કિસ્સામાં, વહેલી તકે નોડલ એજન્સીને જાણ કરીશ.

(III) I/We are furnishing the Declaration of Intent for the enterprise which is a new enterprise as per definition provided the Gujarat Micro, Small and Medium Enterprise (Facilitation of Establishment and Operation) Rules 2019.

(III) મેં/ અમે ઉદ્યોગ માટે ઇરાદાની ઘોષણા રજૂ કરી રહ્યા છીએ જે ગુજરાત માઇક્રો, સ્મોલ એન્ડ મીડિયમ એન્ટરપ્રાઇઝ (સ્થાપના અને ઓપરેશનની સુવિધા) ના નિયમો ૨૦૧૯ ની જોગવાઈ મુજબ એક નવું એન્ટરપ્રાઇઝ છે.

(IV) I/We have not previously availed the Acknowledgement certificate for the land or building or premises as mentioned in 'Location of Manufacturing/Service Activity of Enterprise' in Declaration of Intent.

(IV) ઘોષણા પત્રની તારીખે એન્ટરપ્રાઇઝના ઉત્પાદન/સેવા પ્રવૃત્તિના સ્થળે મુજબ, હું/અમોએ આ અગાઉ આ જમીન અથવા મકાન અથવા પરિસરના માટે સ્વીકૃતિ પ્રમાણપત્ર મેળવેલ નથી.

(V) I/We will maintain the safety standards in operation of the enterprise to ensure safety and health of the workers or employees.

(V) કામદારો અથવા કર્મચારીઓની સલામતી અને આરોગ્યની ખાતરી કરવા માટે હું/અમે એન્ટરપ્રાઇઝના સંચાલનમાં સલામતીના ધોરણોને જાળવીશું.

(VI) I/We will not engage in any manufacturing or service activity which is prohibited under any law or which may pose serious threat to environment, public health or law and order.

(VI) હું અથવા અમે, કે જે કોઈ પણ કાયદા હેઠળ પ્રતિબંધીત હોય અને પર્યાવરણ, જાહેર આરોગ્ય અથવા કાયદો અને વ્યવસ્થા માટે ગંભીર ખતરો પેદા કરી શકે તેવી આઈટમનું ઉત્પાદન/સેવાકીય પ્રવુતિ કરીશું નહિ.
